Job summary

Many Rivers Whole Health in Helena, MT is seeking a dedicated Adult Case Manager to provide recovery-oriented case management for adults with serious mental illness. You will assess needs, develop individualized plans, coordinate services, and advocate for access to benefits, housing, and community supports.

You will spend substantial time in clients' homes and communities, building relationships, ensuring timely documentation, and collaborating with providers and families to promote

Qualifications

• Bachelor's degree in a human services field or 5+ years relevant experience.
• Knowledge of case management principles and community resources.
• Strong verbal and written communication skills.
• Valid Montana driver's license and ability to travel throughout the community.

Responsibilities

• Complete comprehensive assessments and reassessments.
• Develop and maintain individualized treatment plans with clients and partners.
• Monitor progress and ongoing eligibility for services.
• Coordinate services with providers and community resources.
• Advocate for clients and assist with benefits, housing, and entitlements.
• Provide at least 50% of time in client homes and community settings.
• Complete documentation accurately and timely.
• Transport clients when necessary.
• Participate in trainings, meetings, and dev opportunities.
