Job summary

MFS is seeking an Adult Case Manager to deliver community-based case management and care coordination for adults with serious mental illness and co-occurring challenges. The role supports stability, recovery, and access to medical, behavioral health, housing, and social resources.

You will coordinate care across providers, work within an interdisciplinary team, and focus on outreach, documentation, and quality assurance in office, community, residential, hospital, and home-based settings as

Qualifications

• Bachelor's degree in social work, psychology, counseling, rehabilitation, or related field.
• Or an associate degree with relevant experience in behavioral health or community-based services.
• Experience with adults with serious mental illness or co-occurring disorders preferred.
• Ability to work collaboratively within a multidisciplinary team.
• Strong communication, organizational, and interpersonal skills.
• Experience with electronic medical record systems preferred.
