Services occur in clients' communities, homes, workplaces, and social settings, aiming to improve independence and community integration.

The role involves engaging clients, coordinating access to needed services, and ensuring timely referrals and thorough documentation in accordance with SRMHW standards.

Qualifications

• BA/BS degree in human service field or equivalent through experience and education.
• Demonstrated interpersonal skills to work with adults with severe mental illness.
• Background checks and pre-employment drug screening required.

Responsibilities

• Provide goal directed supports and solution-focused interventions for adults with severe mental illness.
• Assist clients and families to identify strategies and treatment options.
• Ensure referrals are acted upon within 3 business days of referral.
• Maintain timely documentation of all services per policy.
• Attend required trainings and workshops for service provision.
