Qualifications

• Bachelor’s Degree in Human Services preferred or with other significant relevant Human Services experience.
• Ability to effectively relate to employees and the public in a positive way.
• Ability to work effectively independently, or in a team with people of all socio-economic classes.
• Produce documents written with clearly organized thoughts using proper sentence construction, punctuation and grammar.
• Have excellent judgement, communication, and conflict resolution skills.
• Have a working knowledge of Center programs, current policies and regulations governing CKMHC operations.

Responsibilities

• Maintain an assigned patient case load with efficient volume of service as well as specific patient outcomes.
• Establish and implement treatment goals and objectives consistent with case management assessment as part of a treatment team and treatment plan.
• Demonstrate an understanding of the Medicaid definition of “Case Management Services” as well as other rehabilitation services.
• Provide face-to-face and administrative services with patients and their families.
• Comply with all documentation standards which are relevant to the provision of care.
• Regular and predicable attendance and punctuality.
