Job summary

Open Sky Community Services is seeking a licensed clinician or licensure-track candidate to provide assessment, treatment planning, and therapeutic support across residential and community settings. The role emphasizes person-centered planning, evidence-based practices, and collaboration with care coordination partners.

Applicants should hold a Master’s in Social Services and licensure in progress (LCSW/LADCI/LMHC) with preferred crisis intervention and residential experience.

Qualifications

• Master’s Degree in Social Services field, required.
• Licensed practitioner or eligible (LCSW/LADCI/LMHC) per hire timeline.
• Experience in crisis intervention and residential work preferred.
• Valid driver’s license with acceptable driving record.

Responsibilities

• Collaborate with care coordination partners, healthcare providers, community agencies, and other stakeholders to ensure individuals receive comprehensive, integrated support.
• Develop and maintain productive partnerships with community-based organizations and service providers to strengthen the agency's network of supports and resources.
• Conduct individual counseling sessions, facilitate support groups, and lead treatment planning meetings with individuals served.
• Provide clinical consultation, guidance, and training to members of the interdisciplinary team.
• Participate in the on-call rotation and critical incident review process, including potential on-site response when needed.
