Job summary

Open Sky Community Services is seeking a licensed clinician or licensure-track clinician to provide assessment, treatment planning, and therapeutic support across residential and community-based settings in Central Massachusetts. You will complete screening, intake, and enrollment activities and develop individualized plans using person-centered and evidence-based practices.

Responsibilities include counseling, treatment planning meetings, and collaborating with a multidisciplinary team.

Qualifications

• Master's Degree in Social Services field required.
• Must be a Licensed Practitioner of the Healing Arts or licensed eligible with LCSW/LADCI/LMHC timelines.
• Experience in crisis intervention and residential work preferred.

Responsibilities

• Collaborate with care coordination partners, healthcare providers, community agencies, and stakeholders to ensure comprehensive support.
• Develop and maintain partnerships with community-based organizations to strengthen the agency's network.
• Conduct individual counseling sessions, facilitate support groups, and lead treatment planning meetings with individuals served.
• Provide clinical consultation, guidance, and training to interdisciplinary team.
• Participate in on-call rotation and potential on-site response when needed.
