Responsibilities

• Collaborate with care coordination partners, healthcare providers, and community agencies to provide integrated support.
• Develop and maintain partnerships with community-based organizations and service providers.
• Conduct individual counseling sessions, facilitate support groups, and lead treatment planning meetings.
• Provide clinical consultation, guidance, and training to interdisciplinary team members.
• Participate in on-call rotation and critical incident review, including on-site response when needed.
