Qualifications

• Master’s degree in Social Services field required.
• Licensure: LCSW within 6 months, LADCI within 1 year, LMHC within 2 years.
• Crisis intervention and residential work experience preferred.

Responsibilities

• Collaborate with care coordination partners, healthcare providers, community agencies, and other stakeholders to ensure individuals receive comprehensive, integrated support.
• Develop and maintain productive partnerships with community-based organizations and service providers to strengthen the agency's network of supports and resources.
• Conduct individual counseling sessions, facilitate support groups, and lead treatment planning meetings with individuals served.
• Provide clinical consultation, guidance, and training to members of the interdisciplinary team.
• Participate in the on-call rotation and critical incident review process, including potential on-site response when needed.
