Qualifications

• Master's Degree in Social Services field required.
• Licensure: LCSW (licensed within 6 months of hire), LADCI (within 1 year), LMHC (within 2 years).
• Crisis intervention and residential work experience preferred.
• Valid driver's license and acceptable driving record required.

Responsibilities

• Collaborate with care coordination partners, healthcare providers, and community agencies to ensure integrated support.
• Develop and maintain partnerships with community-based organizations to strengthen the network of supports.
• Conduct individual counseling sessions, facilitate support groups, and lead treatment planning meetings.
• Provide clinical consultation, guidance, and training to the multidisciplinary team.
• Participate in on-call rotation and critical incident review, including potential on-site response.
