Job summary

Option Care Health, Inc. is seeking a Clinical Transition Educator to provide clinical education and training for seamless patient transitions into our care model.

You will partner with the sales team to expand patient transitions to our care delivery system and coordinate with referral sources. The role requires building relationships with referrals, educating patients and families, and coordinating with clinical and account teams to execute growth plans.

Qualifications

• High School Diploma or equivalent with 2 years in home infusion or post-acute infusion services.
• 1 year of related experience with an associate’s degree or LPN.
• Ability to establish and maintain relationships with diverse stakeholders.
• Familiarity with URAC/ACHC or related standards for relevant services.
• Strong customer service mindset and ability to meet quality standards.

Responsibilities

• Proactively maintains and grows relationships with referral sources to increase sales and patient starts.
• Interacts with area service providers to sell Option Care services for discharged patients.
• Educates patients and families about Option Care services.
• Communicates with agencies and medical teams during discharge to implement treatment plans.
• Collects and reviews preadmission information and secures signoffs.
• Partners with Account Manager and Regional Sales Director on area growth plans.
• Coordinates educational and promotional programs for patients, caregivers, and referrals.
• Facilitates patient transitions to home or ATS and assists with orders transfer.
