Option Care Health, Inc. is seeking a Clinical Transition Educator to educate and train staff for patient transitions into our care delivery model.

You will partner with the sales team to drive patient transitions and collaborate with clinical teams to determine viable candidates for alternative site care. The role emphasizes building relationships with referral sources, educating patients and families, and coordinating with Account Managers to implement growth plans.

Qualifications

• Relationship building with diverse stakeholders.
• Customer service experience across internal/external customers.
• Ability to present information clearly to varied audiences.
• Proficient with MS Office suite (Word/Excel/PowerPoint).
• Willingness and ability to travel to care facilities and sites.

Responsabilités

• Proactively maintains relationships with referral sources to increase sales and patient starts.
• Interacts with area providers to sell services for discharge planning.
• Educates patients and families about Option Care services.
• Communicates with agency staff and medical teams to implement treatment plans.
• Gathers preadmission information and secures signoffs.
