Job summary
Option Care Health, Inc. is seeking a Clinical Transition Educator to provide clinical education and training for patient transitions to our care model.
You will partner with the sales team to increase the number of patients transitioned to OCH care delivery, ensuring seamless home or alternate site care. The role requires an active RN license, at least 2 years in healthcare, strong presentation and relationship-building abilities, and willingness to travel locally to facilities and hospitals to
Qualifications
- Active, unrestricted RN license.
- Minimum 2 years healthcare experience.
- Strong relationship-building and customer service skills.
Responsibilities
- Educate patients, caregivers, and staff on how OCH services are delivered at home or in alternatives.
- Conduct patient assessments to determine eligibility for the OCH care model.
- Collaborate with the sales team to identify educational needs and improve transition volume.
- Maintain confidentiality of patient information and comply with legal privacy standards.
- Document education using OCH applications and technology.
- Coordinate transitions from hospital to home with facilities, providers and internal teams.
- Provide in-person education, collect pre-admission and insurance information, and initiate services.
