Job summary

Option Care Health is seeking a Clinical Transition Specialist to drive transitions from hospital to home care by educating patients, families and facility staff and by building strong referral relationships. You will collaborate with intake, discharge planners and account managers to coordinate services and ensure timely, high‑quality patient transitions.

The role requires an RN or Pharmacist license in the practicing state with at least 2 years in healthcare, plus proficiency with CRM tools

Qualifications

• Licensed RN or Pharmacist in state of practice and at least 2 years of healthcare experience.
• Ability to establish relationships with stakeholders across hospitals, clinics and referral sources.
• Strong communication and presentation skills; able to educate patients and staff.

Responsibilities

• Proactively maintains and grows relationships with referrals sources to increase sales and patient starts.
• Serves as key point of contact and representative to provide education and service to referral sources.
• Interacts with area service providers to sell Option Care services for discharging patients.
• Conducts assessment of patients to ensure suitability for alternative site care.
• Educates patients, families and staff about services and products offered.
• Provides hands-on education to patients and caregivers to support timely discharge.
• Communicates with agency staff, medical team, patients and families during discharge.
• Collects and reviews pre-admission information and obtains signoff.
• Partners with Account Manager to create area business and growth plans.
• Keeps Account Manager informed on opportunities and progress with referrals.
• Reviews medical records to obtain history and insurance payor information for enrollment.
• Initiates care transition coordination with referral sources and internal partners.
• Participates in data collection for therapy start and patient tracking.
• Transfers orders via phone, fax, or e-prescribing when needed.
• Maintains confidentiality of patient and proprietary information and follows laws.
• Provides oversight to providers on patient assessment and home care transition.
• Acts as a contact and coordinator with other providers.
• Makes arrangements for special medical supplies or appliances as needed.
