Qualificações

• RN graduate of an accredited school of nursing.
• NJ State Professional Registered Nurse License and BLS HCP Certification.
• BSN preferred.
• Transfer center experience preferred.

Responsabilidades

• Answers telephone calls according to established policies and procedures.
• Coordinates communication between sending and receiving facilities and within the receiving facility.
• Accurately records details of all transfer-related communications in the electronic health record.
• Collaborates with capacity management, nursing, physician/provider representatives, and hospital departments to assure appropriate care and room assignment.
• Manages bed availability awareness across patient care units and outpatient departments.
• Maintains composure under high workload and interruptions, setting priorities for timely completion.
• Proficiently uses multiple computer applications and telephone systems.
• Implements solutions to transfer center issues and escalates barriers per policy.
• Follows patient identification procedures and collaborates with patient access services as needed.
• Participates in onboarding and mentoring of team members.
