Qualifications

• RN graduate of an accredited school of nursing.
• BSN degree preferred.
• Minimum of 2 years clinical experience in an acute care setting.
• Transfer center experience preferred.
• Critical care and emergency medical services experience preferred.

Responsabilités

• Facilitate communication between sending and receiving facilities.
• Document a record of encounters using electronic health records.
• Coordinate communication within the receiving facility.
• Accurately record details of transfer requests in a timely manner.
• Collaborate with healthcare team members to assure proper level of care.
• Maintain awareness of bed availability across patient care units.
• Handle high-pressure situations and establish priorities for work completion.
• Diffuse challenging situations and respond appropriately.
• Collaborate with Emergency Medical Services (EMS) for patient transportation.
• Initiate registration activities and complete registration as requested.
