Responsibilities

• Deliver nursing care according to current evidence-based policies and standards.
• Coordinate care with interdisciplinary team to ensure safe, effective patient outcomes.
• Document care and patient progress at the point of care using electronic systems.
• Administer medications with bar-code verification and safe medication practices.
• Assist with discharge planning and transitions of care.
• Monitor patient status and respond to changes with appropriate interventions.
