Job summary

(Great Plains Post Acute) Wichita SNF Healthcare, LLC is seeking a Speech-Language Pathologist (SLP) to evaluate and treat patients, communicate with families and physicians, and maintain documentation. You will supervise CFYs and support staff, and participate in care conferences and education sessions.

The role requires a master’s or doctoral degree in SLP, state licensure, and proficiency with Casamba and Point Click Care.

Qualifications

• Master’s or Doctoral degree in Speech-Language Pathology (SLP).
• Licensed as an SLP by the state where services are provided or eligible for licensure.

Responsibilities

• Evaluate patients within 24 hours of referral and document delays.
• Develop and obtain approval for treatment plans from referring physician.
• Treat patients per the plan and supervise CFY/CFY-程度 staff as applicable.
• Train facility staff through inservices and caregiver training.
• Document evaluations, treatment notes, and progress per policies and state acts.
• Assist in discharge planning and family education.
