Job summary

GREATPLAINS in is seeking a Speech-Language Pathologist (SLP) to evaluate and treat patients, communicate with families and physicians, and maintain documentation in medical records. The SLP will supervise CFY and SLP assistants, participate in care conferences, and provide staff training within the facility.

The role requires an SLP license in the state, a master's or doctoral degree in SLP, and proficiency with data entry in Casamba and Point Click Care.

Qualifications

• Master's or doctoral degree in Speech-Language Pathology (SLP).

Responsibilities

• Evaluate patients within 24 hours or 1 business day after referral. Document delays with cause clearly stated in medical record.
• Develop effective treatment plan and obtain approval from referring physician.
• Treat patients per the physician treatment plan.
• Supervise CFY/Speech-Language Pathologist Assistants in direct patient care and related activities.
• Communicate patient progress with supervisor and health team members.
• Participate in Patient Care Conferences, Utilization Review and Rehabilitation Conferences as needed.
• Record evaluations, daily treatment notes, weekly progress notes, and recertification documentation per state and payer requirements.
• Instruct families or nursing staff in maintenance program and caregiver training for discharge.
• Act as clinical preceptor for CFY/RFP SLPs and students.
• Comply with State Practice Act and facility policies.
• Relate positively with patients, families, staff and colleagues.
