Job summary

LiveWell Homecare Agency, LLC seeks a Speech Language Pathologist (SLP) to provide direct speech therapy care to patients in a skilled nursing facility setting. You will assess, establish, and modify functional goals while implementing treatment plans per physician referrals and regulatory standards.

Maintain patient progress documentation, collaborate with families and interdisciplinary teams, and supervise CFYs if applicable.

Qualifications

• Graduate of an accredited speech therapy program (MS/MA in Speech-Language Pathology).
• ASHA certification (CCC) or eligibility for Clinical Fellowship Year (CFY).
• Current state licensure and CPR certification.

Responsibilities

• Assess, establish, and modify functional treatment goals for patients.
• Implement and follow speech therapy treatment plans according to physician referrals and regulatory standards.
• Chart patient progress accurately and timely using therapy software.
• Collaborate with interdisciplinary teams, patients, and families to ensure comprehensive care.
• Oversee and evaluate care provided by Speech-Language Pathology Clinical Fellows (CFYs), if applicable.
• Participate in community health initiatives and maintain professional relationships.
• Adhere to confidentiality, safety, and organizational policies.
• Maintain licensure, certifications, and continuing education requirements.
• Provide patient-centered speech therapy services for diverse patient populations with orthopedic, neurological, cardiopulmonary, and post-acute needs.
• Support patient recovery, functional progress, and quality of life within a supportive environment.
• Participate in compliance, safety, and quality assurance activities.
• Engage in ongoing professional development and community health efforts.
