Job summary

Qualifications

• Bachelor's Degree in Social Work or related field from an accredited college or university.
• Qualified by education, training or experience to work with the adolescent, adult and/or geriatric patient population as specialty assignment dictates.
• Minimum 1 year experience of social work experience.

Responsibilities

• Assesses patient and family needs and determines priorities for social work intervention.
• Documents assessment including social work recommendations and professional judgment within 24 business hours of referral.
• Coordinates discharge planning early with the multidisciplinary team and community resources.
• Provides crisis intervention services to patients and families and arranges appropriate referrals.
• Maintains collaboration with collateral agencies to support patient care.
