Job summary

Indiana University Health in Bloomington Hospital is seeking a Social Worker to assess psychosocial needs and connect patients and families with support programs. You will interview patients and relatives to gather social history relevant to medical conditions and care planning.

As part of a multidisciplinary team, you will help remove barriers to care, coordinate discharge planning, and link patients to community resources, ensuring safe and effective care transitions.

Qualifications

• Bachelor's Degree in Social Work (BSW) required.
• Minimum of 3 years in healthcare, social work, or related fields.
• Permanent Indiana LBSW licensure required for all new hires effective October 5, 2025; incumbents must obtain licensure within 6 months of hire; incumbents with LSW must maintain licensure.

Responsibilities

• Conduct face-to-face interviews with patients and families to assess social history and psychosocial needs.
• Identify barriers to care and assist patients in accessing social support programs and community resources.
• Serve as a liaison between healthcare providers, patients, families, and outside agencies to facilitate communication and support.
• Provide solution-focused assistance related to medical conditions and hospitalization.
• Collaborate with multidisciplinary teams to develop discharge plans and resource development strategies.
