You will perform psychosocial assessments and help coordinate discharge plans within a multidisciplinary hospital team. Required: BS in a health-related field, MSW coursework or a master’s degree in social work (or equivalent), and acute care social work experience preferred.

Qualifications

• BS in health-related field.
• Graduate curriculum leading to a master’s degree in social work or equivalent education, training and/or experience.

Responsabilités

• Provides psychosocial assessments on an as-needed basis, and in accordance with Case Management Department Policies and Procedures.
• Promotes and adheres to IRMC core values and patient care standards.
