Regional One Health is seeking a Social Worker (Bachelor's) to provide social work services and counseling for patients and families in collaboration with the physician and health care team. You will arrange timely referrals for psychosocial assessment as appropriate.

The role emphasizes discharge planning, coordination with multidisciplinary staff, and maintaining documentation and regulatory compliance. A focus on adolescent, adult, and geriatric populations is expected as assigned.

Competenze

• Bachelor's Degree required in Social Work or related field from an accredited college or university.
• Minimum 1 year of social work experience preferred.

Mansioni

• Assess patients and families; determine priorities for social work intervention.
• Coordinate discharge planning with the care team.
• Provide crisis intervention and referrals as needed.
• Document assessments within 24 business hours of referral.
• Collaborate with community resources and collateral agencies.
