Responsabilités

• Perform social work assessment and develop plans with patient/family input.
• Coordinate discharge planning and communicate with multi-disciplinary team.
• Provide crisis intervention services to patients and families as needed.
• Document assessments, plans, and recommendations within 24 business hours of referral.
• Coordinate referrals to community resources and services (DME, therapy, home care, etc.).
• Maintain confidentiality and adhere to HIPAA, iRESPECT, and regulatory requirements.
