Qualifications

• Bachelor's Degree in Social Work or related field from an accredited college or university.
• Qualified by education, training or experience to work with adolescent, adult and/or geriatric patient populations as assignment dictates.
• Minimum 1 year experience in social work preferred.

Responsabilités

• Performs social work assessments and provides psychosocial services.
• Coordinates discharge planning and collaborates with the care team for timely referrals.
• Provides crisis intervention and appropriate referrals based on assessment findings.
• Documents assessments and recommendations within 24 business hours of referral.
• Maintains patient and family education, and coordinates with community resources.
