• Reports sentinel events or incidents as warranted and documents appropriately.
• Provides education to patients and families and documents in the EHR in a timely manner.
• Initiates discharge planning on high-risk patients and follows up as needed.
• Provides linkage to community resources and services such as Medicaid, housing, and social supports.
