Job summary

IU Health Bloomington Hospital is seeking a Social Worker to assess psychosocial needs, interview patients and families, and coordinate discharge planning. You will connect patients with community resources and support programs, while collaborating with the care team to overcome barriers to care.

The ideal candidate holds a Bachelor's in Social Work (BSW) with at least 3 years in healthcare, and Indiana LBSW licensure by Oct 5, 2025 (or within 6 months of hire).

Qualifications

• Bachelor's Degree in Social Work (BSW) required.
• Minimum of 3 years in healthcare, social work, or related fields.
• Indiana LBSW licensure required for all new hires by Oct 5, 2025; those hired before must obtain licensure within 6 months; LSW licensure must be maintained.

Responsibilities

• Conduct face-to-face interviews with patients and families to assess social history and psychosocial needs.
• Identify barriers to care and assist patients in accessing social support programs and community resources.
• Liaise between healthcare providers, patients, families, and outside agencies to facilitate communication and support.
• Provide solution-focused assistance related to medical conditions and hospitalization.
• Collaborate with multidisciplinary teams to develop discharge plans and resource development strategies.
