Job summary

Indiana University Health is seeking a part-time Social Worker at Fort Wayne Hospital to provide direct, face-to-face support for patients and families. The role partners with the care team to coordinate safe, timely care and to guide complex discharge planning, including involvement in abuse, neglect, and domestic violence cases.

Qualifications

• MSW degree is required.
• Must hold permanent LSW licensure.
• CPI certification or alternative de-escalation training required within 6 months of hire.
• Experience with psychosocial assessment and crisis intervention.
• Knowledge of discharge planning processes and social determinants of health.

Responsibilities

• Meet face-to-face with patients and families to assess needs and coordinate care.
• Collaborate with the health care team to ensure safe, timely episodes of care and provide resources.
• Interprete and communicate complex patient/family needs to other caregivers.
• Assess psychosocial risk factors and evaluate support systems and coping ability.
• Counsel patients and families on emotional, social, and financial implications of illness.
• Mobilize family and community resources to meet identified needs.
• Provide mental health assessments when referred and crisis intervention as needed.
• Assist with complex discharge planning and safety planning.
• Act as liaison to protective services and guardianship/adoption processes.
• Offer specialized mental health services and crisis intervention as required.
