Job summary

Oak Street Health is seeking a Medical Scribe to support primary care providers by documenting patient encounters in real time. You will observe visits, assign CPT/ICD-10 codes, and help prepare After Visit Summaries to ensure accurate, timely records.

Ideal candidates have medical terminology knowledge, some clinical exposure, and strong listening while adapting to new workflows. This full-time role is in-clinic, with standard hours and career growth opportunities in health informatics.

Qualifications

• Knowledge of medical terminology and common medications.
• Prior clinical experience such as shadowing or volunteering preferred.
• Scribe or transcription experience preferred but not required.

Responsibilities

• Document patient encounters in a structured note (history, assessment, plan, exam).
• Assign CPT and ICD-10 codes.
• Prepare After Visit Summaries and ensure documentation accuracy.
• Support daily huddles and clinical documentation reviews.
• Provide administrative support to the care team.
