Job summary

Oak Street Health is seeking a Medical Scribe to support primary care providers with real-time documentation during patient visits. You will observe visits in the exam room, document encounters, assign CPT and ICD-10 codes, and prepare after-visit summaries to ensure accurate records.

Qualifications

• Knowledge of medical terminology and common medications.
• Prior clinical experience, including shadowing and/or volunteering.
• Prior scribe or transcription experience (preferred)

Responsibilities

• Documenting patient encounters in a structured note with history, assessment, plan and exam.
• Assigning CPT and ICD-10 codes; preparing After Visit Summaries.
• Supporting clinical documentation improvement and reviewing medical records.
• Administrative tasks to assist the care team and provider.
