Job summary

Oak Street Health is seeking a Medical Scribe to support primary care providers with real-time documentation. You will observe visits, document encounters, and help prepare After Visit Summaries to improve patient care.

Ideal candidates have clinical exposure or pre-med experience, strong listening skills, and fluency in language(s) spoken by community patients. Full-time hours with comprehensive benefits and on-site clinic work are included.

Qualifications

• Knowledge of medical terminology and common medications is required.
• Prior clinical experience, including shadowing or volunteering, is strongly preferred.
• Scribe or transcription experience is preferred but not required.
• Advanced listening and communication skills are required.
• Strong computer literacy and ability to learn new workflows are required.
• Fluency in Spanish, Polish, Russian, or related languages is required where indicated.

Responsibilities

• Document patient encounters in a structured note with HPI, assessment, plan and exam.
• Join providers in exam rooms to observe visits.
• Assign CPT and ICD-10 codes.
• Prepare After Visit Summaries for patients.
• Consult with providers to ensure accurate documentation.
• Support clinical documentation improvement and population health tools.
• Attend daily huddles and clinical documentation reviews.
• Provide administrative support to the care team (orders and referrals).
• Adhere to HIPAA and clinic policies.
• Work with the team to handle additional documentation duties as assigned.
