UTMB Health seeks a Clinical Practice Pharmacist to provide advanced medication therapy management and patient-focused care across practice areas. The role emphasizes evidence-based decision making, collaboration with physicians and nurses, and contributing to standardized pharmacy services across sites.

Responsibilities include optimizing therapy, educating patients, and participating in rounds, audits, and quality improvement initiatives in a dynamic healthcare setting.

Qualifications

• Bachelor of Science in Pharmacy or Doctor of Pharmacy degree.
• Completion of an ASHP accredited PGY-1 residency or 3 years related experience.
• Licensed to practice pharmacy in Texas or eligible; must obtain licensure within 90 days of hire.
• Preferred: Doctor of Pharmacy degree.

Responsabilités

• Provide advanced medication therapy management and direct or consultative patient care.
• Participate in interprofessional care teams and apply evidence-based guidelines.
• Manage medication therapy to optimize outcomes and cost-effectiveness.
• Collaborate with physicians, nurses, case managers, and other professionals for coordinated care.
• Educate patients on medications, adherence, and potential side effects.
• Contribute to implementation of clinical pathways, protocols, and regulatory standards.
• Support medication-use evaluations, quality improvement, and safety initiatives.
• Precept learners and participate in experiential education activities.
