UTMB Health in Galveston, TX seeks a Clinical Practice Pharmacist to provide advanced medication therapy management and direct patient care across inpatient and ambulatory settings.

The role combines clinical decision-making with operational awareness to ensure safe, effective, and cost-conscious medication use and collaborates with physicians, nurses, and other professionals to optimize patient outcomes and participate in education and quality improvements.

Qualificações

• Bachelor of Science in Pharmacy or Doctor of Pharmacy degree.
• Completion of an ASHP accredited post-graduate year 1 residency program or 3 years of related experience.
• Licensed to practice pharmacy in the state of Texas or eligible for licensure from the Texas State Board of Pharmacy.
• Must be licensed with the Texas State Board of Pharmacy within 90 days of hire.
• Doctor of Pharmacy degree (PharmD) preferred.
• Completion of an ASHP accredited post-graduate year 2 residency program a plus.

Responsabilidades

• Assess medication regimens, identify and resolve therapy-related problems, and provide recommendations to optimize clinical outcomes and cost-effectiveness.
• Monitor and evaluate therapeutic responses and adverse events, adjusting therapy per protocol or with prescribers.
• Implement evidence-based guidelines, internal policies, and external requirements to ensure appropriate therapy selection.
• Partner with physicians, nurses, case managers, and other professionals to support coordinated, patient-centered care.
• Participate in patient care rounds, case conferences, and interdepartmental meetings as assigned.
• Serve as a clinical resource for drug information and policy interpretation.

Formação académica

Bachelor of Science in Pharmacy or Doctor of Pharmacy degree

ASHP residency PGY1 or 3 years related experience

Licensure in Texas or eligibility for licensure

Doctor of Pharmacy degree (PharmD)
