Job summary

Qualifications

• PharmD or BS in Pharmacy required for consideration.
• Completion of ASHP PGY1 residency or 3 years of related experience.
• TX license or eligibility with 90 days to obtain licensure.

Responsibilities

• Provide advanced medication therapy management and direct patient care.
• Participate in interprofessional rounds and care planning.
• Implement evidence-based guidelines and policy interpretations.
• Collaborate with physicians, nurses, and other healthcare professionals to optimize outcomes.
• Educate patients on medications and adherence.
• Support medication-use evaluations and quality improvement initiatives.
