Qualifications

• Bachelor of Science in Pharmacy or Doctor of Pharmacy degree.
• Completion of an ASHP accredited post-graduate year 1 residency program or 3 years of related experience.
• Preferred: Doctor of Pharmacy degree.
• Preferred: ASHP post-graduate year 2 residency for specialty areas or job.
• Preferred: Specialty Pharmacy experience.

Responsibilities

• Assess and optimize medication therapy to improve outcomes and cost-effectiveness.
• Collaborate with physicians, nurses, and other clinicians for patient-centered care.
• Adhere to evidence-based guidelines, policies, and payer/regulatory requirements.
• Educate patients about medications, adherence, and potential side effects.
• Participate in rounds, case conferences, and interdepartmental meetings.
• Provide drug information and clinical decision support to staff.
• Support medication-use evaluations and quality improvement initiatives.
• Assist in developing clinical pathways and protocol-driven services.
