Job summary

UTMB Health in Galveston, TX seeks a Clinical Practice Pharmacist to deliver advanced medication therapy management and direct patient care across assigned services. The role integrates clinical decisions with operational awareness to ensure safe and cost-conscious medication use aligned with system goals.

The pharmacist collaborates with physicians, nurses, and other healthcare professionals, participates in rounds, and supports quality improvement initiatives.

Qualifications

• Doctor of Pharmacy or Bachelor of Science in Pharmacy.
• ASHP-accredited PGY-1 residency or 3 years related experience required.
• Licensed to practice in Texas or eligible for licensure by the Texas State Board of Pharmacy.
• Must be licensed with the Texas State Board of Pharmacy within 90 days of hire.
• Preferred: Doctor of Pharmacy degree.
• Completion of ASHP-accredited PGY-2 residency is a plus.

Responsibilities

• Assess and optimize medication therapies across practice areas.
• Monitor responses and adverse events; adjust therapy per protocol.
• Implement guidelines, policies, and payer/regulatory requirements.
• Collaborate with physicians, nurses, and other professionals for patient care.
• Participate in rounds, conferences, and interdepartmental meetings.
• Provide drug information and evidence-based recommendations to clinicians.
• Support medication-use across distribution with operations input.
• Identify and report medication-use system safety issues.
• Educate patients on medications, adherence, and side effects.
• Precept learners and contribute to experiential education.
