Job summary

UTMB Health in Texas City seeks a Clinical Practice Pharmacist to provide advanced medication therapy management and direct patient care across inpatient and ambulatory settings. Qualifications

• Bachelor of Science in Pharmacy or Doctor of Pharmacy degree.
• ASHP-accredited PGY1 residency or 3 years of related experience.
• Texas licensure to practice pharmacy or eligibility; licensure within 90 days of hire.

Responsibilities

• Assisting with transplant pharmacy referrals.
• Assisting with transitions of care.
• Assisting with patient education.
• Assess medication regimens and optimize clinical outcomes and cost-effectiveness.
• Monitor therapeutic responses and adverse events; adjust therapy per protocol or with prescribers.
• Implement evidence-based guidelines and ensure regulatory compliance.
• Collaborate with physicians, nurses, and other health professionals.
• Participate in patient care rounds and interdepartmental meetings.
• Provide drug information and evidence-based recommendations to clinicians.
• Support safe medication distribution by addressing workflow and inventory issues.
• Educate patients on medications and adherence.
• Precept pharmacy learners and participate in IPPE/APPE activities.
