Job summary

UTMB Health is seeking a Clinical Practice Pharmacist to provide advanced medication therapy management across inpatient and ambulatory settings. You will participate in interprofessional teams, apply guidelines, and optimize therapy to balance safety and cost.

Responsibilities include rounds, decision support, patient education, and contributing to clinical pathways while ensuring compliance with accreditation and regulatory standards.

Qualifications

• Bachelor or PharmD degree required.
• ASHP PGY-1 residency or 3 years related experience required.
• Texas licensure or eligible for licensure within 90 days of hire.

Responsibilities

• Assess medication regimens and optimize outcomes.
• Monitor therapies and adverse events; adjust per protocol.
• Implement guidelines and payer/regulatory requirements for safe, cost-effective therapy.
• Collaborate with physicians, nurses, and case managers for patient-centered care.
• Participate in patient care rounds, case conferences, and interdepartmental meetings.
• Serve as a clinical drug information resource for the team.
• Support safe medication distribution by addressing workflow and inventory issues.
• Educate patients on medications, administration, side effects, and adherence.
• Develop and refine clinical pathways and protocol-driven services.
• Ensure compliance with accreditation and regulatory standards (TJC, CMS, USP, state board).
