Job summary

UTMB Health seeks a Clinical Practice Pharmacist to provide advanced medication therapy management and patient-centered care across multiple practice settings, including inpatient and ambulatory services.

You will collaborate with physicians, nurses, and pharmacists to optimize therapy, adhere to evidence-based guidelines, and support quality and safety initiatives across sites of care.

Qualifications

• Bachelor of Science in Pharmacy or Doctor of Pharmacy degree.
• Completion of an ASHP-accredited PGY1 residency or 3 years of related experience.

Responsibilities

• Assess and optimize medication regimens for efficacy and cost-effectiveness.
• Monitor responses and adverse events; adjust therapy per protocol or with prescribers.
• Implement guidelines and policies to ensure safe, appropriate therapy.
• Collaborate with physicians, nurses, and other health professionals for patient-centered care.
• Participate in rounds, case conferences, and interdepartmental meetings.
