Job summary

UTMB Health in Galveston, TX seeks a Clinical Practice Pharmacist to provide advanced medication therapy management and direct patient care across inpatient and ambulatory settings. The role blends clinical decision‑making with operational awareness to ensure safe, effective, and cost‑conscious therapy.

You will join interprofessional teams, apply evidence‑based guidelines, educate patients, monitor responses, and participate in pharmacotherapy initiatives, pathways, and quality projects.

Qualifications

• Bachelor of Science in Pharmacy or Doctor of Pharmacy degree.
• Completion of ASHP accredited PGY1 or 3 years related experience.
• Licensed to practice pharmacy in Texas or eligible; must obtain licensure within 90 days of hire.

Responsibilities

• Assess medication regimens, identify and resolve therapy-related problems, and provide recommendations to optimize clinical outcomes and cost-effectiveness.
• Monitor and evaluate therapeutic responses and adverse events, adjusting therapy per protocol or in collaboration with prescribers.
• Educate patients on medications, including how to take them correctly, potential side effects, and adherence importance.
• Collaborate with physicians, nurses, case managers, and other healthcare professionals to support coordinated, patient-centered care.
• Participate in patient care rounds, case conferences, and interdepartmental meetings as assigned.
• Support safe and efficient medication distribution by addressing workflow, product selection, and inventory issues.
