UTMB Health is seeking a Clinical Practice Pharmacist to provide advanced medication therapy management and direct or consultative patient care across multiple practice areas. The role emphasizes clinical decision-making, safety, and cost-conscious therapy in alignment with system goals.

The position requires Texas pharmacy licensure within 90 days of hire, with ASHP‑accredited residency training or equivalent experience, and supports participation in interprofessional teams and quality

Qualifikationen

• Bachelor of Science in Pharmacy or Doctor of Pharmacy degree.
• Completion of an ASHP accredited post-graduate year 1 residency program or 3 years of related experience.
• Preferred: Doctor of Pharmacy degree.
• Completion of an ASHP accredited post-graduate year 2 residency program for specialty areas or job.

Aufgaben

• Assess medication regimens, identify and resolve therapy-related problems, and provide recommendations to optimize outcomes.
• Monitor therapeutic responses and adverse events, adjusting therapy per protocol or with prescriber input.
• Implement evidence-based guidelines, policies, and payer or regulatory requirements to ensure safe, cost-effective therapy.
• Collaborate with physicians, nurses, case managers, and other clinicians for coordinated patient care.
• Participate in patient care rounds, conferences, and interdepartmental meetings as assigned.
• Provide drug information, policy interpretation, and evidence-based recommendations to healthcare professionals.
