Job summary

Healthfirst is seeking a Pharmacy Technician to act as a Pharmacy Navigator, reviewing medication lists and removing barriers to therapy to improve adherence and prevent adverse events. The role coordinates refills, new prescriptions, appointments, and access to medications for patients in financial need.

Qualifications

• Minimum qualification includes a high school diploma or GED from an accredited institution.
• Experience communicating with patients, providers, and pharmacists on medication and related issues.
• Nationally certified Pharmacy Technician (PTCB/NHA) or licensed/registered Pharmacy Technician in good standing required.

Responsibilities

• Review medication lists to understand and remove barriers to therapy and support adherence as the member's Pharmacy Navigator.
• Facilitate refills, new prescriptions, appointments, and access to medications, including for financial issues, to minimize pharmacy visits.
• Coordinate MTM interventions with Healthfirst pharmacists and internal/external partners.
• Monitor member adherence through coordination with community pharmacies.
• Conduct patient health assessments to identify barriers to adherence related to medications and access to care.
• Handle member and provider communications telephonically and in person when needed.
• Perform additional duties as necessary.
