Job summary

PAM Health is seeking a Clinical Navigator to be the face of our hospital, cultivating referral relationships and managing a territory while meeting patients at their toughest times. You will analyze referral and admission data and collaborate with admissions and nursing staff to facilitate referrals and improve patient satisfaction.

The role involves developing relationships with physicians, discharge planners, and payer representatives, and contributing to program development and quality

Qualifications

• Education and Training: Degree in a health-related field from an accredited college or university preferred.
• Experience: Minimum of two years’ experience in the designated field of license, preferably with rehabilitation/LTACH and marketing experience.

Responsibilities

• Cultivates referral relationships, manages assigned territory, and completes patient assessments.
• Makes sales contacts as well as analysis of referral and admission data from assigned territory.
• Works with Admissions Manager and staff to facilitate referral conversion and improve customer service, referral and admission processes.
