PAM Health is hiring a Clinical Navigator to act as the face of our hospital, cultivating referral relationships and guiding patients through the rehab process. You will work with the Director of Business Development and Admissions staff to optimize referrals and admissions while maintaining strong collaborations with nurses, physicians, and payers.

Competenze

• Degree in a health-related field from an accredited college or university preferred.
• Current licensure in nursing, respiratory therapy, physical therapy, occupational therapy, or speech and language therapy is required.
• Prior marketing and/or rehabilitation/LTACH experience preferred.

Mansioni

• Cultivates referral relationships, manages assigned territory, and completes patient assessments.
• Makes sales contacts as well as analysis of referral and admission data from assigned territory.
• Works with Admissions Manager and staff to facilitate referral conversion and maintain stakeholders relationships.
