PAM Health seeks a Clinical Navigator to cultivate referral relationships, manage a territory, and assess patients. You will collaborate with admissions staff, nursing, and external partners to improve referrals and admissions.

The role emphasizes building durable relationships with clinicians and payers to support patient transitions. The position requires a health-related degree, active licensure, and at least two years in the field, ideally with rehab LTACH or marketing background.

Formación

• Education and Training: Degree in a health-related field from an accredited college or university preferred.
• Current licensure in nursing, respiratory therapy, physical therapy, occupational therapy, or speech and language therapy is required.
• Prior marketing and/or rehabilitation/LTACH experience preferred.

Responsabilidades

• Cultivates referral relationships and manages assigned territory.
• Completes patient assessments and makes sales contacts.
• Analyzes referral and admission data with the team to improve outcomes.
• Builds relationships with physicians, nurses, discharge planners, and payer reps.
