Job summary

PAM Health Specialty and Rehabilitation Hospital of Covington is seeking a Clinical Navigator to cultivate referral relationships, manage an assigned territory, and complete patient assessments. You will work with the Admissions Manager and nursing staff to optimize referrals and admissions across the facility.

The role focuses on building strong relationships with physicians, discharge planners, and payers while driving customer satisfaction and program improvements.

Qualifications

• Degree in a health-related field from an accredited college or university preferred
• Current licensure in nursing, respiratory therapy, physical therapy, occupational therapy, or speech and language therapy is required
• Prior marketing and/or rehabilitation/LTACH experience preferred

Responsibilities

• Works under the direction of the Director of Business Development to cultivate referral relationships and manage assigned territory
• Completes patient assessments and makes sales contacts; analyzes referral and admission data
• Collaborates with Admissions Manager, admission staff, nursing, and external staff to facilitate referral conversion
• Provides feedback for program development and quality improvement initiatives related to customer service and patient satisfaction
• Develops and maintains relationships with clinicians, discharge planners, case managers and payer representatives
