Job summary

PAM Health is seeking a Clinical Navigator to be the face of our hospital and to cultivate referral relationships while managing an assigned territory and completing patient assessments.

You will make sales contacts, analyze referral/admission data, and collaborate with the Admissions Manager, nursing staff, and payer representatives to improve referrals and admissions. This role supports program development and quality initiatives across the organization.

Qualifications

• Degree in a health-related field is preferred; current licensure required.
• Minimum two years' experience in the designated field, preferably rehab/LTACH and marketing.
• Licensure in nursing, respiratory therapy, physical therapy, occupational therapy, or speech-language pathology is required.

Responsibilities

• Cultivate referral relationships and manage assigned territory.
• Complete patient assessments and make sales contacts to facilitate referrals.
• Analyze referral and admission data and collaborate with Admissions Manager and nursing staff.
• Support program development and quality improvement initiatives related to customer service and admissions.
