Job summary

PAM Health Dallas seeks a Clinical Navigator to drive referral development and patient admissions within an assigned territory. You will build relationships with physicians, discharge planners, and payer representatives while coordinating with admissions and nursing staff to optimize the referral and admission process.

The role emphasizes patient-centered care, data-driven improvement, and collaboration across departments to support rehabilitation goals.

Qualifications

• Degree in a health-related field from an accredited college or university.
• Current licensure in nursing, respiratory therapy, physical therapy, occupational therapy, or speech and language therapy.
• Prior marketing and/or rehabilitation/LTACH experience preferred.
• Minimum of two years’ experience in the designated field of license.

Responsibilities

• Cultivates referral relationships and manages assigned territory.
• Completes patient assessments and makes sales contacts.
• Analyzes referral and admission data to drive conversion.
• Collaborates with Admissions Manager and nursing staff to improve processes.
• Provides feedback for program development and quality improvement.
