Job summary

PAM Health is seeking a Clinical Navigator to be the face of our hospital, meeting patients at difficult times and guiding them through rehab stay with support and encouragement.

You will cultivate referral relationships, manage a defined territory, and collaborate with Admissions and nursing teams to optimize referrals and admissions. We value proactive feedback to improve programs and patient satisfaction.

Qualifications

• Degree in a health-related field from an accredited college or university preferred.
• Current licensure in nursing, respiratory therapy, physical therapy, occupational therapy, or speech and language therapy is required.
• Minimum of two years’ experience in the designated field, preferably with rehabilitation/LTACH and marketing experience.

Responsibilities

• Cultivates referral relationships and manages assigned territory.
• Conducts patient assessments and makes sales contacts.
• Collaborates with admissions staff and nursing to facilitate referral conversion.
• Provides feedback for program development and quality improvement in customer service and referral processes.
