Job summary

PAM Health is seeking a Clinical Navigator to serve as the face of the hospital, cultivating referral relationships and managing a designated territory within the Victoria area. The role requires engaging with physicians, discharge planners, and payer representatives to optimize referrals and admissions.

You will work under the Director of Business Development, contributing to program development, quality improvement, and patient-centric care throughout the rehabilitation stay.

Qualifications

• Degree in a health-related field from an accredited college or university preferred.
• Current licensure in nursing, respiratory therapy, physical therapy, occupational therapy, or speech and language therapy is required.
• Prior marketing and/or rehabilitation/LTACH experience preferred.
• Minimum of two years’ experience in the designated field of license.

Responsibilities

• Cultivates referral relationships and manages assigned territory.
• Completes patient assessments and makes sales contacts.
• Analyzes referral and admission data from the territory.
• Collaborates with admissions and nursing staff to facilitate referrals and admissions.
• Provides feedback for program development and quality improvement.
• Maintains relationships with patients, families, physicians, discharge planners, case managers and payer representatives.
